5 Phases of completion  

PHASE 1 

Responsibilities 
  • Learn staff’s names
  • Learn group member’s names
  • Read handbook
  • Pass a written test with an 85% or higher score
  • Complete all chores and work projects in a timely manner
  • Show respect to self, peers, staff and visitors
  • Introduce yourself in a formal group setting and in treatment team
  • Participate in group and individual counseling
Privileges

  • Can play board games
  • Can play sports at staff’s discretion
  • Can watch television during rec time at staff’s discretion

Phase 2

Responsibilities
  • Meet with worker and counselor to implement treatment plan
  • Complete time line and life story
  • Show respect to self, peers, staff and visitors
  • Show improvement in behavior and in following rules
  • Use proper table manners
  • Begin work on ITP goals
  • Participate in group and individual counseling
  • Follow group agreements in group settings
Privileges
  • Can use all rec equipment and participate in all rec activities at staff’s discretion
  • May buy soft drinks and candy on Friday, Saturday and Sunday at designated times at staff’s discretion
  • Can work with staff on special work projects on campus

Phase 3

responsibilities
  • Complete Gene-O-Gram
  • Reduce behavior problems
  • Reduce number of rule infractions and redirections by staff
  • Learn to express feelings and emotions in proper ways
  • Show improvement and continue to work on goals
  • Review minimum of two goals in individual counseling, group counseling and with staff
  • Show care and concern for other residents by helping them with their problems
  • Participate in group and individual counseling
privileges
  • Can use all rec equipment and participate in all rec activities at staff’s discretion
  • May buy sodas and candy at designated times with staff’s permission
  • Can work off campus with staff to earn money but not during school hours unless on work study.
  • Eligible for work study off campus
  • Additional privileges that apply from previous phases

PHASE 4

RESPONSIBILITIES
  • Participate in group and individual counseling
  • Show respect to self, peers, staff and visitors
  • Minimal redirections for behavior and rule infractions
  • Take responsibility for your behavior
  • Begin to self discipline
  • Continue to work and show progress on all ITP goals
  • Review a minimum of two ITP goals in individual and group counseling and with staff
  • Complete relapse prevention plan process in individual counseling
privileges
  • All privileges from phase three
  • Eligible for one furlough*

Phase 5

responsibilities 
  • Be a mentor for lower phases
  • Follow all rules fo the facility
  • Show respect for self, peers, staff and visitors
  • Meet with worker for discharge plan
  • Complete all remaining school work
  • Show noticeable changes in all goal areas
  • Review remaining ITP goals with staff in individual and group counseling
  • Process relapse prevention plan in group counseling and with staff
  • Volunteer to help others with chores, school work and in the program
privileges
  • All privileges from phase four
  • Eligible for one furlough*
*The 2 weekend furloughs are to help with re-entering back into the home and community once the resident leaves Foothills Academy.
